I. Introduction to Optics & Laser Lab

II. Optics & Laser Lab Access Guidelines

Access and use of this room require the following:

A.  Complete the UC Learning Center e-course "Laboratory Safety for Research Personnel"
B.  Complete the UC Learning Center e-course "Laser Safety Training"
C.  Orientation to the BE-148 Lab & Surroundings.
D.  Adding you to the LHAT roster and your completion of LHAT for the BE-148 Optics and Laser Lab

III. How to Get Access and Process

Step 1:   Email:   bels-request@soe.ucsc.edu
Subject:   BE-148 Optics & Laser Lab Access - <your name>
Body:   Please state why you need access and the faculty member you are working under.   If you have already completed all or some of the training listed above, please state this.  Note that work not part of a course that may involve a lab access fee.  This fee can be paid by a Student Misc Fee (billed to the student) or by recharge (billed to UCSC FOAPAL).

Step 2:  Safety Training

A.  Complete the UC Learning Center e-course "Laboratory Safety for Research Personnel" 
All personnel accessing the lab must be current in the UC required "Laboratory Safety for Research Personnel" training. Training is good for three years and then must be refreshed. The LHAT system will track your UC training courses. You can log in and check your current status at will.

B.  Complete the UC Learning Center e-course "Laser Safety Training"  
Please complete UCSC Laser Safety Training and take one of the laser safety quizzes.  Provide evidence of completion of the laser safety course to BELS via the request tracker ticket created. 

C.   Lab Orientation
TA's will give an orientation to the lab.

D.  Certify the Hazard Assessment for the Fabrication Lab on the Lab Hazard Assessment Tool (LHAT) 
All personnel accessing the Instructional Laser & Optics Lab must have certified the hazard assessment on LHAT for this space. The hazard assessment is reviewed and updated periodically and you will need to recertify the assessment when this update occurs in order to continue to have access to the room. Those who are on the LHAT roster at the time of an update to a hazard assessment will be contacted automatically about the update.

Step 3:  BELS staff will request via ADCKEYS system for you to have access to the BE-148 Instructional Optics and Laser Lab. (Update: TAs will provide access to the labs. Students will not have access outside of scheduled lab times.)

 IV.  Checkoff List for Access

1.  Optics & Laser Lab Orientation (note a)
2.  Complete the UC Learning Center e-course "Laboratory Safety for Research Personnel"
3.  Certify the Hazard Assessment for the Instructional Laser Lab on the Lab Hazard Assessment Tool (LHAT) 
4.  LHAT PPE Certification
5.  Complete the UC Learning Center e-course "Laser Safety Training"
6.  If using a laser, read and sign appropriate SOP
